Hospice Eligibility Guidlines

Eligible Conditions

Conditions that are generally eligible for hospice care include:

  • Adult failure to thrive
  • Dementia
  • Chronic gegenerative neurologic disease
  • Heart disease
  • HIV/AIDS
  • Huntington’s disease
  • Liver disease
  • Parkinson’s disease
  • Pulmonary disease
  • Renal failure
  • Stroke
  • Coma

Eligibility Criteria

  • Terminal condition with estimated life expectancy of six months or less if the illness runs its normal course
  • Philosophical shift from curative to palliative care
  • Family member or caretaker willing to assume 24-hour responsibility of care when safety becomes an issue in being home alone
  • Patient and/or family agrees to hospice care
  • Physician referral
