Hospice Eligibility Guidlines

Hospice eligible conditions include:

  • Adult failure to thrive
  • Dementia
  • Chronic gegenerative neurologic disease
  • Heart disease
  • HIV/AIDS
  • Huntington’s disease
  • Liver disease
  • Parkinson’s disease
  • Pulmonary disease
  • Renal failure
  • Stroke
  • Coma

General hospice criteria:

  • Terminal condition with estimated life expectancy of six months or less if the illness runs its normal course
  • Philosophical shift from curative to palliative care
  • Family member or caretaker willing to assume 24 hour responsibility of care when safety becomes an issue in being home alone
  • Patient and/or family agrees to hospice care
  • Physician referral

Remember – Hospice is a 6-month service, not a 6-hour service. Our goal is to assist the patient and their loved ones to be as comfortable, aware and as involved as possible throughout this difficult time. Hospice care neither hastens nor postpones death. Our interdisciplinary team places emphasis on providing care that will meet the physical, emotional and spiritual needs of the patient and family.
