Welcome Incoming Residents and Fellows!
Welcome to Texas A&M Health Science Center College of Medicine at Scott & White (TAMHSC-COM/S&W). We are glad to have you as part of our family. These pages contain all of the information you need to get started.
Step One: Know your Program Administrator
Your residency coordinator is the administrative contact for your residency program and is here to assure that you have an optimal educational experience. He/She may have additional program-specific items for you to complete. Please bring your final diploma to orientation, your residency coordinator will need to review it.
Step Two: Your Health
To safeguard your health, all residents/fellows are required to bring documentation of
- Tetanus/diphtheria or TDAP immunization within the past 10 years.
- Two doses of MMR vaccine or proof of immunity to measles (rubeola) and rubella
- PPD will be performed during orientation. If you have had a positive PPD, then you need to present a copy of a chest X-ray report taken after the PPD was positive.
- Completion of the Hepatitis B series and a Hepatitis B titer done after completion of the series.
- Two doses of the varicella immunization or titer stating immunity or verbalization of having chicken pox as a child.

To facilitate a seamless entry into your role as a resident/fellow at Scott & White and our major participating institution, please complete all hospital forms for Scott & White and Central Texas Veterans Healthcare System (CTVHCS).

Central Texas Veteran Healthcare System (CTVHCS) Forms
All CTVHCS forms need to be completed, signed, dated and returned at least six weeks prior to the start of orientation. If a section does not apply, please enter N/A in that blank. Any questions regarding the CTVHCS forms should be directed to Nancy Beimer.

Step Four: Obtain your Physician-in-Training Permit and your National Provider Identifier (NPI)
Physician-in-Training Permit
You will need to apply for your Physician-in-Training permit online through Texas Medical Board (TMB):
NOTE: TMB suggests that you print each screen, as well as the review screen, of your application as it is completed for your records. You will not be able to save your application. Please apply for your permit between April 1 - May 1, 2009 (early April preferable). Be sure to include an email address for TMB correspondence.

National Provider Identifier (NPI)
You will need to apply for your National Provider Identifier online through The National Plan and Provider Enumeration System (NPPES).
NOTE: For Health Care Providers
The Administrative Simplification provisions of the Health Insurance Portability and Accountability Act of 1996 (HIPAA) mandated the adoption of a standard unique identifier for health care providers. The National Plan and Provider Enumeration System (NPPES) collects identifying information on health care providers and assigns each a unique National Provider Identifier (NPI).
Step Five: Orientation and Training Schedule
Orientation
Orientation will commence on June 26, 2009 at 7 a.m. in the Conference Center Room A200 A and B.
Continental breakfast will be available at 6:45 a.m.

HLC Training Modules – Web-based and need to be completed prior to start of orientation.
