Blood Donor T-Shirt Request Order Form |
This is an order request form. Cost of all t-shirts are $5. Sizes and styles are limited. A blood center employee will contact you within 1-2 business days to verify your order can be fulfilled. Payments may be made in person at the Blood Center in cash or check form.
Scott & White employees may send a check through interoffice at their own risk. A blood center employee will contact you to verify payment arrival prior to mailing your order. The blood center is not responsible for any lost or stolen payments or t-shirts through the mail.
