How to Make a Referral

Here is the minimum information required to make referral:

• Patient's name
• Patient's phone number
• Patient's diagnosis
• Patient's date of birth
• Patient's physician's name
• Name and phone number of the person making the referral

Anyone may contact Scott & White Hospice to refer someone for hospice service. However, the patient/family must agree to hospice care and a physician must order the care.
