Getting Started with Scott & White

If you are a new referring physician or an existing referring physician who needs to change your contact information, you can print and fax a referring physician information form , or fill out the secure online form.

Please have your National Provider Identifier (NPI) and Federal Tax ID numbers ready. For some health care providers, the Unique Provider Identification number (UPIN) is still used.

When you refer a patient to Scott & White, we strive to make sure:

• Your patient will receive a timely appointment.
• Our physicians will contact you about your patient's condition and recommended treatment options.
• As a referring physician, you will be satisfied with the level of service both you and your patient receives.
• The appropriate physician for your patient is selected. Review our physician's directory to learn more about our physicians and qualifications.
