To request a shot record from The Children's Hospital at Scott & White, download the following form and fill in the appropriate information. You may fax your completed request form to 254-724-0391, or deliver it to the Ambulatory Pediatric Clinic.

Are your child's vaccinations up-to-date? Download the Center for Disease Control (CDC) recommended childhood and adolescent immunization schedule. This schedule lists the ages (birth to 18 years old) for when each vaccine or series of shots is to be given. If your child has missed any shots, check with your doctor about getting back on track. © CDC
