The minimum information required to make referral:
- Patient's name
- Patient's phone number
- Patient's diagnosis
- Patient's date of birth
- Patient's physician's name
- Name and phone number of the person making the referral
Anyone may contact Scott & White Hospice to refer someone for hospice service. However, the patient/family must agree to hospice care and a physician must order the care.
