About the Program

The Children's Specialty Care Coordination Program (CSCCP) provides medical, surgical, habilitative care and psycho-social support to identified populations of children with chronic and disabling conditions grouped by diagnosis.

The CSCCP is a unique program that provides care coordination to children enrolled in the interdisciplinary teams.  Healthcare for children on our teams is complicated because of the array of physicians, nurses and therapists involved in decision-making and treatment.  The teams, composed of medical staff members, allied health and social service personnel from the clinic and hospital, are dedicated to the unique interdisciplinary needs of each group of children.

One day a year, the child will be evaluated by the entire team, which takes approximately four hours  and saves parents multiple trips to the hospital. The child will see every specialist who will evaluate them individually and then join together to build a treatment plan to be carried out through the year. During that year, the team will see that every patient is following through with the team recommendations.

The policy for the team is one of open-door. The coordinator acts a liaison to the doctors for the family and remains involved throughout the process. Children are followed through with from birth to the age of 21 and then aided in transitioning to adult care if need be.

Most interdisciplinary medical services are provided on the main Scott & White campus in Temple with routine follow-up care provided whenever possible near the child's home by local professionals. Some of the team members provide periodic consultation in some Scott & White satellite clinics to provide specialty services close to the patients' homes. A satellite Cleft Palate Team has been established at Scott & White in College Station. The satellie team is composed of physicians, dentists and speech pathologists from the community, as well as personnel from College Station and Temple Scott & White clinics.
